STATMENTS:

1. Please completely fill this Return Merchandize Authorization (RMA) form and fax it with original purchase invoice to EVOC Intelligent to indicate a valid RMA number before returning goods.
2. RMA number will be void after 14 days of the ISSUE date. Extra payment will be charged if the RMA return goods is/are out of warranty.
3. Please attach this RMA FORM and a copy of ORIGINAL PURCHASE INVOICE legibly on the top of your return goods. Return goods without RMA form and purchase invoice will not be accepted.
4. All P/N codes of RMA product must fully match the purchase invoice.
5. EVOC Intelligent will not be responsible, or liable thereafter in respect of this RMA, and reserve all the rights without prior notice.
